This is so heartbreaking and yet hopeful all at once.
You do a really great job of getting across the state of physical and inner turmoil that Sylar is going in. Stuck in this endless cycle of Hell and clinging to the one thing that makes it possible to keep going...but even that is slipping away.
I really like that even when Mohinder shows up Sylar is skeptical and disbelieving...refusing to let himself fall for such a cruel trick.
It's a very touching scene at the end with Mohinder embracing Sylar and letting him release all those hurts, all that pain...almost literally trying to shoulder it from him.
